Course description | This is a core course in year 2 of the Bachelor of Nursing with Honours (and Bachelor of Nursing Ordinary degree) programme. The course is suitable for health and social care professionals who have undertaken Healthy Communities 1. The course content is delivered through a series of lectures and tutorials. Its focus is on the health of communities as part of a larger society. It builds on the students¿ knowledge and understanding of the impact which community and family dynamics can have on the health of an individual. Utilising taught theories and techniques, students are enabled to develop skills to start to assess and address health and social care needs of communities. The course addresses:
* Assessing the needs of a community
* Community development and enablement
* Health inequalities and social inclusion
* The principles of public health
Communication and reflection skills introduced in year 1 are further developed and students consider the challenges when communicating with communities e.g. literacy, confidentiality and diversity.
The course provides opportunities for the students to apply their learning in both the academic and clinical environment. To progress through the course students are required to produce evidence of a community profile and a community health needs assessment that they have undertaken for a defined community. Students also propose a health improvement activity to meet the needs they have identified in the community profile and needs assessment. These activities enable students to:
1)Demonstrate an understanding of how people¿s lifestyles, environments and the location in which they live influence health and
2)Apply their knowledge and become competent in translating the community profile and needs assessment into an action plan for health improvement for the benefit of people within communities. |

* Are aware of current health and social care policies and legislation
* Understand the influence which policy has on the health and social care services targeting communities
* Identify the important role which health improvement activities have in changing the health of communities
* Understand the various community approaches employed by health professionals to improve the public¿s health and wellbeing
* Have knowledge about how to support and work in partnership with individuals, families and communities who are affected by disability, mental health issues and long term conditions.
* Have an understanding about enabling individuals to self-care when living with a long term condition
* Have knowledge about the complexities of working with various communities including young people, older people and those who are ethnically diverse
* Build on knowledge about the role of health and social care practitioners in safeguarding children and adults and consider this when developing community interventions

Assessment Information
Formative - Oral and visual presentation which is peer assessed.
Summative - Production of a community profile, confined to within 2,500 ¿ 3,000 words, for a defined community, health needs assessment plus development of an action plan, and health improvement activity.
